Resumen Introducción: La bronquiolitis obliterante postinfecciosa es una enfermedad pulmonar poco frecuente; existen limitados reportes en Sudamérica. Caso clínico: En esta serie se reportan 10 pacientes con esta enfermedad diagnosticados en el Instituto Nacional de Salud del Niño-Breña (Lima-Perú). La mediana de edad al diagnóstico fue de 19 meses. Todos los pacientes presentaron el antecedente de infección respiratoria aguda grave. Los síntomas más frecuentes fueron tos, dificultad respiratoria, sibilancias e hipoxemia; el patrón de atenuación en mosaico fue la característica más frecuente en la tomografía. Todos tenían serología positiva para adenovirus. Se administró tratamiento con pulsos de metilprednisolona, azitromicina, hidroxicloroquina y corticoides inhalados. Ningún paciente falleció durante el seguimiento. Conclusiones: En los niños previamente sanos con antecedente de infección respiratoria aguda grave y sintomatología obstructivo bronquial persistente se debe considerar el diagnóstico de bronquiolitis obliterante postinfecciosa. Este es el primer reporte en Perú con un régimen terapéutico adaptado a nuestra institución.


Abstract Background: Postinfectious bronchiolitis obliterans is a rare lung disease; there are limited reports in South America. Case report: We report 10 patients with this disease diagnosed at the Instituto Nacional de Salud del Niño-Breña (Lima-Peru). The median age at diagnosis was 19 months and all patients had a history of severe acute respiratory infection. The most frequent symptoms were cough, respiratory distress, wheezing, and hypoxemia. The mosaic attenuation pattern was the most frequent on the tomography. All the patients had positive serology for adenovirus. The treatment received was methylprednisolone pulses, azithromycin, hydroxychloroquine, and inhaled corticosteroids. No patient died during the follow-up. Conclusions: In previously healthy children with a history of severe acute respiratory infection and persistent bronchial obstructive symptoms, the diagnosis of postinfectious bronchiolitis obliterans should be considered. This is the first report in Peru with a therapeutic regimen adapted to our institution.

ABSTRACT

Objective:To investigate the clinical characteristics and prognosis of human adenovirus (HAdV) infection in patients with allogeneic hematopoietic stem cell transplantation (allo-HSCT).Methods:This is a retrospective case series study. Patients who received allo-HSCT and had symptoms of HAdV infection were tested in the Hematology Department at Perking University People′s Hospital from August 2015 to October 2019. Real-time quantitative PCR was used to detect HAdV DNA from 2 728 patients with potential infection. HAdV DNA-positive patients were defined as having HAdV infection. The clinical features of these patients were analyzed, and a case-pair method was used to select patients without HAdV infection as the control group in a 1∶3 ratio. The clinical results of the two groups were compared using Kaplan-Meier and Log-rank testing.Results:A total of 7 119 samples were tested for HAdV, of which 99 samples from 36 patients were positive. Of these patients, 22 developed HAdV viremia, and 24 patients had concurrent infection with another virus. Nineteen patients had fever (53%), 25 had gastrointestinal symptoms (69%), 11 had respiratory symptoms (31%), nine had reduced liver function (25%), and six had nervous system symptoms (17%). Twenty-three patients developed acute graft-versus-host disease of grade 2 or higher. Of all the patients with HAdV infection, nine were treated with cidofovir, seven of whom became HAdV negative and two had invalid treatment. The median follow-up time was 496 (216, 940) d post-HSCT. The overall survival at 5 years post HSCT was 48.4%±9.2% vs. 91.3%±3.5% ( χ2=65.03, P<0.001) in patients with and without HADV, respectively. The non-relapse mortality at 5 years post-HSCT was 40.8%±8.8% vs. 4.0%±2.0% ( χ2=34.17, P<0.001) in patients with and without HADV, respectively. Conclusions:After allo-HSCT, HAdV-infected patients are dominated by gastrointestinal and respiratory symptoms and have an increased risk of combined acute graft-versus-host disease of >2 degrees. Patients with HAdV infection have poor overall survival and high non-relapse mortality.

ABSTRACT

Objective:To investigate the clinical features of adenoviral encephalitis (AE), and to provide reference for clinical diagnosis and treatment of adenoviral encephalitis.Methods:From January 2012 to December 2020, 1 185 cerebrospinal fluid (CSF) samples of hospitalized children with suspected central nervous system infection in the Second Affiliated Hospital of Shantou University Medical College were collected for the detection of 22 common respiratory pathogens and common pathogens for encephalitis by polymerase chain reaction. Records of patients with adenovirus positive in CSF were reviewed and relevant clinical manifestations, laboratory tests and imaging examination results were collected for analysis.Results:Among 1 185 CSF samples, 242 samples were positive for viral nucleic acid, with detection rate of 20.4%, including 1.9%(23/1 185) of adenovirus. As for 23 children diagnosed with AE, 18 were male, five were female, with the age of (44.8±35.9) months, ranging from two months and 19 days to 10 years. Of 23 children, 21(91.3%) presented with fever, followed by convulsions (16 cases, 69.6%), headache (four cases, 17.4%), vomiting (11 cases, 47.8%), consciousness change (11 cases, 47.8%) and emotion disturbance (three cases, 13.0%). Among 23 children, eight cases had white blood cell counts (WBC) of (6 to <10)×10 9/L, 10 cases had WBC of (10 to 20)×10 9/L and the white blood cell classification was mainly neutrophils (21 cases, 91.3%), and C reactive protein of 20 cases (87.0%) was in the normal range. Cerebrospinal fluid examination showed that WBC were less than 15×10 6/L in 20 cases (87.0%), and WBC ≥15×10 6/L in three cases, which were up to 500×10 6/L; the protein of 19 cases was in the normal range, the glucose of 15 cases was in the normal range, and the chloride of 19 cases was in the normal range. Among 16 cases with brain magnetic resonance imaging examination, eight cases did not show abnormality, six cases with local meningeal linear enhancement, one case with small intracranial malacia, and one case with extensive intracranial lesions. For 13 cases who received electroencephalogram (EEG) test, seven cases showed normal EEG or marginal state, four cases showed extensive medium and high amplitude slow wave, one case showed spike wave or spike slow wave and one case had both of the above two changes. Among 23 children, 22 cases recovered including one case had secondary epilepsy, and the remaining one case had severe brain dysfunction and was unable to suck when discharged, with an indwelling gastric tube and accompanied by secondary epilepsy. Conclusions:The clinical manifestations and auxiliary examinations of children with AE have no obvious specificity. Most children with AE have a good prognosis, but a small number of them may have serious sequelae.

Objective:To classify human adenovirus (HAdV) of adenoviral conjunctivitis by sequencing and phylogenetic analysis of hexon protein and fiber protein.Methods:A total of 256 conjunctival swabs were collected from the inferior conjunctival sac of 256 patients with viral conjunctivitis in Shanghai from January 2015 to August 2017.After DNA extraction, the whole length of hexon and fiber was amplified by PCR to perform gene sequencing and phylogenetic analysis.The study protocol was approved by the Ethics Committee of Shanghai General Hospital (No.2020-202).Written informed consent was obtained from each subject.Results:Among the samples, 89(34.76%) were positive for hexon gene amplification, including 1(1.12%) of HAdV-C1, 7(7.87%) of HAdV-C2, 20(22.47%) of HAdV-B3, 6(6.747%) of HAdV-E4, 23(25.84%) of HAdV-D8, 17(19.10%) of HAdV-D19, and 15(16.85%) of HAdV-D37.In phylogenetic analysis, sequenced hexon gene was clustered with the reference prototype correctly.In fiber phylogenetic analysis, 15 of HAdV-D19 and 1 of HAdV-D37 were cross clustered.Conclusions:The combined sequencing of hexon and fiber can provide abundant and effective biological information for the subtype and pathogenicity analysis of HAdV.

ABSTRACT

Objective:To analyze the clinical characteristics of severe adenovirus pneumonia with bacterial infection in children, so as to provide clinical evidences for early diagnosis and reliable treatment.Methods:A total of 72 pediatric cases with severe adenovirus pneumonia admitted to Pediatric Intensive Care Unit at the First People′s Hospital of Chenzhou from January 2018 to August 2019 were included.The clinical features, laboratory and imaging data, efficacy of the treatments and prognosis were investigated retrospectively.Patients were divided into with bacterial infection group and without bacterial infection group.The clinical characteristics were analyzed.Results:Among the 72 children, there were 54 males and 18 females, aging from 3 months to 5 years, including 37cases with bacterial infection and 35 cases without bacterial infection.Compared with the group without bacterial infection, the group with bacterial infection had longer heat duration and hospital stay [12.0 (10.0, 18.5) days vs.10.0 (9.0, 12.0) days; 6.0(4.0, 7.0) days vs.11.0(6.5, 16.0) days, P<0.05], the incidences of diarrhea and hepatomegaly were higher[35.1% (13/37) vs.14.3%(5/35); 45.9%(17/37)vs.8.6%(3/35), P<0.05], the proportion of five lobes involved in lung imaging was higher [91.8% (34/37) vs. 57.1%(20/35), P<0.05]. The incidences of complications with respiratory failure, hemophagocytic lymphohistiocytosis, and bronchiolitis obliterans in the group with bacterial infection were significantly higher than those of without bacterial infection group ( P<0.05). Conclusion:For children under 2 years old age with severe adenovirus pneumonia, there are prolonged high fever and extensive pulmonary lesions.We should be highly alert to the combination of bacterial infection and timely anti-infection therapy.Children with severe adenovirus pneumonia with bacterial infection have severe clinical manifestations and many complications with respiratory failure, hemophagocytic lymphohistiocytosis, and bronchiolitis obliterans.

Human adenoviruses (HAdVs) are the common infectious pathogens in children, which mainly infect the respiratory tract, digestive tract and eyes.Severe HAdVs may even be life-threatening.HAdVs enter the host and cause infection by binding to the host epithelial cells.At the same time, they are recognized by various immune cells and then activate the immune defense response.This study aims to review the interaction between HAdVs and the host, and the immune defense mechanism, thus improving the understanding of the immune response to HAdVs.

ABSTRACT

The recombinant adenoviruses expressing miR-22 (Ad-miR-22) was constructed and the effect of Ad-miR-22 on insulin signal pathway and glucose uptake in HepG2 cells was analyzed. MiR-22 gene was amplified by PCR from human hepatocytes and cloned into the pAdTrack-CMV vector to generate the shuttle plasmid pAdT-22. The positive colonies were confirmed by PCR and sequencing. The resultant shuttle plasmid was linearized with Pme I, followed by co-transformation into competent BJ5183 cells containing an adenoviral backbone plasmid (pAdEasy-1) to create the recombinant plasmid pAd-miR-22. After digested with Pac I, the linearized pAd-miR-22 was transfected into 293A packaging cell line to generate recombinant adenoviruses Ad-miR-22. HepG2 cells were infected with Ad-miR-22 or control Ad-GFP (adenoviruses expressing green fluorescent protein), and then the miR-22 expression levels were analyzed by qPCR. The result shows that adenovirus-mediated overexpression of miR-22 significantly decreased insulin-induced glucose uptake in HepG2 cells. Moreover, overexpression of miR-22 markedly decreased insulin-induced phosphorylation of GSK-3β. miR-22 also increased the mRNA levels of gluconeogenic genes in HepG2 cells. Furthermore, Western blotting results indicate that the protein expression of SIRT1 decreased in Ad-miR-22 infected HepG2 cells as compared with Ad-GFP infected HepG2 cells. In summary, overexpressing of miR-22 significantly increased gluconeogenesis while decreased glucose uptake in HepG2 cells. The effect of miR-22 on glucose metabolism may be mediated by SIRT1.

ABSTRACT Objective: To report the case of a child who developed acute respiratory distress syndrome (ARDS) from a pulmonary infection by adenovirus. Case description: A female patient aged 2 years and 6 months, weighting 10,295 grams developed fever, productive cough and vomiting, later on progressing to ARDS despite initial therapy in accordance with the institutional protocol for ARDS treatment. The child evolved to refractory hypoxemia and hypercapnia, requiring high parameters of mechanical pulmonary ventilation and use of vasoactive agents. In the treatment escalation, the patient received steroids, inhaled nitric oxide (iNO), was submitted to the prone position, started oscillatory high-frequency ventilation (HFOV) and extracorporeal membrane oxygenation (ECMO) was indicated due to severe refractory hypoxemia. During this time, the patient's clinical response was favorable to HFOV, improving oxygenation index and hypercapnia, allowing the reduction of vasoactive medications and mechanical ventilation parameters, and then the indication of ECMO was suspended. The patient was discharged after 26 days of hospital stay without respiratory or neurological sequelae. Comments: Adenovirus infections occur mainly in infants and children under 5 years of age and represent 2 to 5% of respiratory diseases among pediatric patients. Although most children with adenovirus develop a mild upper respiratory tract disease, more severe cases can occur. ARDS is a serious pulmonary inflammatory process with alveolar damage and hypoxemic respiratory failure; Adenovirus pneumonia in children may manifest as severe pulmonary morbidity and respiratory failure that may require prolonged mechanical ventilation. Exclusive pulmonary recruitment and HFOV are advantageous therapeutic options.


RESUMO Objetivo: Descrever paciente que evoluiu com síndrome do desconforto respiratório agudo (SDRA) a partir de infecção pulmonar por adenovírus. Descrição do caso: Paciente de dois anos e seis meses, sexo feminino, peso de 10295 g, que apresentou com quadro de febre, tosse produtiva e vômitos, evoluindo para SDRA. Apesar da terapêutica inicial em conformidade com o protocolo institucional de tratamento da SDRA, a criança evoluiu para hipoxemia e hipercapnia refratárias, necessitando de elevados parâmetros de ventilação pulmonar mecânica e utilização de agentes vasoativos. No escalonamento da terapêutica, a paciente recebeu terapias adjuvantes, foi iniciada ventilação oscilatória de alta frequência (VOAF) e indicada oxigenação por membrana extracorpórea (OMEC) pela hipoxemia grave refratária. Nesse ínterim, a paciente apresentou resposta clínica favorável à VOAF, melhorando do quadro ventilatório e possibilitando a redução das medicações vasoativas e dos parâmetros de ventilação mecânica. A paciente recebeu alta hospitalar após 26 dias de internação, sem sequelas respiratórias ou neurológicas. Comentários: As infecções por adenovírus ocorrem principalmente em lactentes e crianças com menos de cinco anos de idade e representam de 2 a 5% das doenças respiratórias entre os pacientes pediátricos. Embora a maioria das crianças com infecção por adenovírus desenvolva doença leve do trato respiratório superior, casos mais graves podem ocorrer com comprometimento do trato respiratório inferior. A pneumonia por adenovírus em crianças pode se manifestar com morbidade pulmonar grave e insuficiência respiratória com risco de vida, o que resulta na necessidade de suporte mecânico prolongado. O recrutamento pulmonar exclusivo pela VOAF pode ser uma opção terapêutica útil.

OBJECTIVE@#To explore infection rate of different adeno-associated virus (AAV) on knee joint cartilage in mice and to find a good gene editing tool for mice chondrocytes of knee joint.@*METHODS@#Forty-five 4-week-old SPF C57BL/6 weighed(14.3±0.2) g were selected. According to different injections(6 μl) for right knee joint, mice were divided into 9 different groups, 5 mice in each group. The groups were such as following:control group (normal saline), Vigene 2 group (AAV2 from vigene biosciences, titer for 1×10¹³ vg/ml), Vigene 5 group (AAV5 from vigene biosciences, titer for 1×10¹³ vg/ml), Vigene 6 group (AAV6 from vigene biosciences, titer for 1×10¹³ vg/ml), Vigene 7 group (AAV7 from vigene biosciences, titer for 1×10¹³ vg/ml), Vigene 8 group (AAV8 from vigene biosciences, titer for 1×10¹³ vg/ml), Vigene 9 group (AAV9 from vigene biosciences, titer for 1×10¹³ vg/ml), Hanbio DJ group(AAV2-DJ from Hanbio, titer for 1×10¹² vg/ml), Hanbio 5 group (AAV5 from Hanbio, titer for 1×10¹² vg/ml). All AAVs were over-expressed green fluorescent protein(GFP). Knee joint specimens were taken and observed injury of cartilage under stereomicroscope at 30 days after injection, then 10 μm thick frozen sections were prepared. Distribution of green fluorescent protein of meniscus and cartilage of knee joint was observed under fluorescence microscope.@*RESULTS@#Stereomicroscope observation indicated that no obvious lesion was observed in knee joint cartilage of mice after intra-articular injection of AAV. According to frozen sections of knee joints, strong green fluorescence was observed in knee joint cartilage in all AAV experimental groups. Compared with other groups, significantly stronger green fluorescence were observed both in AAV2 and AAV7 groups, whose average fluorescence density was 0.077±0.020 and 0.061±0.022. There were significant differences between two groups and other groups.@*CONCLUSIONS@#AAV could infect chondrocyte of knee joint in vivo by injecting into knee joint cavity. Higher infection efficiency of AAV2 and AAV7 on knee joint cartilage were observed. Local injection of AAV into knee joint cavity could be used as an effective tool for gene editing of knee joint chondrocyte.

The detailed and advanced chest radiographic features of adenovirus pneumonia in children was described.The most frequently dominant imaging pattern adenovirus pneumonia in children was bilateral fused patchy parenchymal opacities or consolidation with a peribronchovascular distribution and bilateral lung hyperinflation.Severe adenovirus pneumonia may be associated with complications and sequela such as bronchiolitis obliterans.Presence of dominant consolidation findings during the early stage in adenovirus pneumonia may be a noteworthy finding for differentiating this condition from other types of viral pneumonia.A better understanding of the typical radiographic findings of adenoviral pneumonia in children may help physicians identify infections at an earlier stage and elucidate the natural course of the disease.

ABSTRACT

In order to explore the possibility of human adenovirus infection with tree shrews,the neutralizing antibody ti-ters of five kinds of human adenoviruses (HAdv)in the serum of tree shrews were analyzed.The levels of Ad3,Ad4,Ad7, Ad14 and Ad55 neutralizing antibody were detected by virus neutralization test.The results showed that the positive rate of four adenoviruses in group B were higher than Ad4 in group E,and the positive rates respectively were Ad14 (55.88%),Ad3 (47.06%),Ad55 (29.71%),Ad7 (14.71%)and Ad4 (8.82%).The antiserum mainly mixed with Ad3,Ad14 and Ad55 anti-body.Five species of human adenovirus can be naturally infected with tree shrews.Tree shrews are used as experimental ani-mals to establish human adenovirus infection model is alternative.

In recent years , respiratory tract infectious diseases and public health events caused by human adenovirus type 7(HADV-7) had been rising.Especially in Asia, the outbreaks of acute respiratory illness caused by HADV-7, accounting for more than 60%in the world, had brought great influence to the public health.In order to prevent the occurrence of HADV-7 epidemic and improve the diagnostic performance of laboratory , the research progress of HADV biological features and typing , HADV-7 epidemiological characteristics , clinical and differential diagnosis and laboratory examination methodologies were reviewed in this paper.

ABSTRACT

Prostate cancer is a common malignant disease in elder men, and it is the second leading death cause in male cancer patients. The incidence rate of prostate cancer has been increasing year by year, and it is ranked the third in the incidence rate of urologic cancers in China. The oncolytic adenovirus carrying specific promoter has been used as a novel target of antitumor therapy. The adenovirus has specific effects on tumor cells and can express downstream therapeutic genes to inhibit the growth of tumor cells, but it can't influence the normal cells or only has weak influence. At present, oncolytic adenovirus carrying specific promoter which has been used widely in basic and clinical studies has showed unique advantages in great antitumor effect and medication safety. This paper reviews the several biological molecules, which have been found during the construction of oncolytic adenovirus, may potentially become to be prostate cancer-specific promoters.

Com o desenvolvimento humano, os recursos hídricos sofrem modificações derivadas da expansão demográfica e principalmente da falta de saneamento básico, que interferem diretamente na qualidade desses recursos com a introdução de substâncias químicas tóxicas e bioagentes patogênicos. Os vírus entéricos de veiculação hídrica, como adenovírus e rotavírus, merecem destaque considerando seus impactos na saúde pública. O objetivo deste estudo foi avaliar a presença de adenovírus e rotavírus em águas superficiais do Córrego Ribeirão Preto, SP. A coleta foi realizada em 11 pontos distintos, da nascente até a confluência com o Rio Pardo. O Teste de ELISA foi o método utilizado para a detecção dos vírus (RIDASCREEN® Adenovirus, RIDASCREEN® Rotavirus - R-Biopharm). De acordo com os resultados obtidos, 73% das amostras foram positivas para adenovírus, enquanto que 36% foram positivas para rotavírus. Considerando a virulência desses vírus entéricos e a vulnerabilidade de indivíduos imunossuprimidos, idosos e crianças, é preocupante a contaminação por esses agentes patogênicos de veiculação hídrica amplamente disseminados em águas superficiais.


Water resources have been suffering with the changes in demographic expansion and the lack of basic sanitation. These factors directly interfere with the quality of these resources due to the discharge of toxic chemicals and pathogenic bio-agents. The waterborne enteric viruses, such as rotavirus and adenovirus, deserve attention considering its impact on public health. The aim of this study was to evaluate the presence of adenovirus and rotavirus in surface water of Ribeirão Preto stream. Samplings were conducted in 11 different points from the spring to the confluence with the Rio Pardo. The ELISA test was carried out for the detection of viruses (RIDASCREEN® adenovirus, rotavirus RIDASCREEN® - R-Biopharm). According to the results, 73% of the samples were positive for adenovirus, while 36% were positive for rotavirus. Given the virulence of these enteric viruses and the vulnerability of immunosuppressed individuals, i.e. the elderly and children, contamination with these waterborne pathogens widely disseminated in surface water is concerning

Las diarreas son una de las principales causas de enfermedad infantil en todo el mundo, especialmente en países en vías desarrollo, en donde las enfermedades diarreicas representan un factor muy importante de mortalidad infantil, alcanzando, principalmente, a menores de 5 años de edad. Se calcula que los adenovirus pueden ser el segundo agente causal de las diarreas agudas después del rotavirus y que contribuye entre el 5 y 20% de los niños hospitalizados por diarrea. El objetivo del estudio fue detectar la presencia de adenovirus entéricos 40 y 41, utilizando la técnica de PCR en muestras de heces diarreicas de 190 niños de ambos sexos, menores de 5 años de edad, de diferentes municipios y centros de salud del estado Zulia, recolectadas en el periodo de Enero de 2011 a Noviembre 2013. Se encontró un 22,10% (42/190) de positividad en las muestras estudiadas, lo que demostró que este virus es causa de gran parte de los casos de diarreas en los niños. Según la procedencia el mayor número de casos estuvo en el Municipio Jesús Enrique Lossada con un 28,57%. Adenovirus predominó en el grupo lactante menor con un 52,38% (22/127), con diferencias significativas (p<0,05) con el resto de los grupos etarios. El sexo masculino fue el más afectado con un 57,14% (24/92) con respecto al femenino. La relación en cuanto a positividad para el virus y las manifestaciones clínicas, se destacan, diarrea en 42/42 (100%), seguido de vómitos 28/42 (73,80%). El presente estudio indica que adenovirus 40 y 41 son agentes etiológicos importantes de las diarreas en la población infantil estudiada.


Diarrhea is one of the main causes of childhood illness worldwide, especially in developing countries, where diarrheal diseases represent a very important factor in infant mortality, reaching primarily, those under 5 years. It is estimated that adenoviruses can be the second causal agent for acute diarrhea after rotaviruses and that they contribute between 5 and 20% of the children hospitalized for diarrhea. The aim of this study was to detect the presence of enteric adenoviruses 40 and 41 using the PCR technique in diarrheal stool samples of 190 children of both sexes under 5 years old. Different municipalities and health centres in Zulia collected samples from January, 2011, to November, 2013. The study found 22.10% (42/190) positive reactions in the samples studied, demonstrating that this virus causes a great part of the diarrheal cases in children. According to source, the highest number of cases was in the Jesus Enrique Lossada municipality with 28.57%. Adenovirus dominated the nursing infant group with 52.38% (22/127), showing significant differences (p<0.05) when compared to other age groups. Males were the most affected with 57.14% (24/92) compared to females. The ratio in terms of virus positivity and clinical manifestations evidenced diarrhea in 42/42 (100%), followed by vomiting 28/42 (73.80%). The present study indicates that adenoviruses 40 and 41 are important etiological agents for diarrhea in the child population studied.

About the malignant tumor in China,the hepatocellular carcinoma mortality is second only to lung cancer and serious threat to the life and health of the masses.Furthermore,because most patients has been in advanced cancer during medical treatment,so had lost the chance of one-stage surgical resection.However,the sensitivity of hepatocellular carcinoma to chemotherapy,radiotherapy and other treatments are poor.Transcatheter arterial chemoembolization is the main method of the treatment for patients those have lost the chance of operation,though the clinical effect is significant,the inadequate is also presence,such as tumor necrosis,incompletely clear,residual tumor nidus and the damage of the immune function after operation.Recombinant adenovirus p53 gene can validly infect tumor cells,transcription and expression of p53 protein,it also regulate the expression of related genes inhibiting tumor cell growth and induce cell apoptosis directly or indirectly.It is increasingly highly attention that recombincanting rAd-p53 with transcatheter arterial chemoembolization(TACE) to treat hepatocellular carcinoma.In order to evaluate the clinical value and promising future,we will make a brief summary for the research progress in this area in recent years.

ABSTRACT

Objective To study the antiviral effects of Pudilan xiaoyan oral liquid on Hep-2 cell models infected with respiratory syncytial virus (RSV), adenoviruses serotype 3 strains (ADV3) in vitro. Methods The cell cytotoxic and inhibition effect of Pudilan xiaoyan oral liquid on RSV or ADV3 were investigated by MTT assay and the inhibitory assay for cytopathic effect (CPE) in Hep-2 cell cultures to detect its antiviral effects. Results The median toxic concentration (TC50) of Pudilan xiaoyan oral liquid on Hep-2 cells was 776.97 mg/L. The median effective concentration (EC50) of inhibiting RSV and ADV3 were 28.08, 28.10 mg/L,whose therapeutic index (TI) were 27.67 and 27.65 respectively. The safety coefficient of Pudilan xiaoyan oral liquid is higher than that of control, ribavirin. Compared with the virus control group, Pudilan xiaoyan oral liquid can obviously produce actions of a dose-dependent relationship to inhibit CPE in Hep-2 cells infected with RSV or ADV3 virus. Conclusions Pudilan xiaoyan oral liquid significantly improves the protection against RSV and ADV3 virus infection in Hep-2 cells. And the inhibition of CPE induced by viruses infection increased with the elevation of higher drug concentration for its antiviral effect augmented in vitro.

Objective To compare the efficacy and incidence of complications of stenting alone and intratumoral injection of recombinant human adenovirus p53 combined with stent implantation in the treatment of advanced esophageal cancer patients.Methods 2 4 cases with advanced esophageal cancer,who could only eat semi liquid,liquid food or could not eat,were randomly divided into control group(A group) and treatment group (B group).12 cases in A group were treated with esophageal stent implantation,12 cases in B group were given endoscopic intratumor injection of recombinant human adenovirus p53 combined with food tract stent implantation.Results The rate of improvement of dysphasia had no significant difference in survival period:the treatment group survival time of 3.2-38 months,mean (27.0 ± 2.3) months,while the control group survival time of 2.8-27months,an average of (14.7 ± 8.0) months,the difference between the two groups was significant (t =3.521,P < 0.05).The incidence rate of restenosis in treatrnent group was 8.3%,that in control group was 41.7%.There was significant difference (x2 =7.532,P <0.05).Conclusion Endoscopic intratumor injection of recombinant human adenovirus p53 combined with esophageal stent implantation in the treatment of advanced esophageal carcinoma can prolong the survival time of patients with esophageal stent implantation,and to reduce the incidence of restenosis.

[ ABSTRACT] AIM: To investigate the seroprevalence of neutralizing antibodies to human adenovirus type 5 (AdHu5) , human adenovirus type 26 (AdHu26) and chimpanzee adenovirus type 68 (AdC68) in the patients with chro-nic hepatitis B ( CHB) and the patients with primary liver cancer ( PLC) , and to provide guidance for developing safe and effective biotherapy vectors against CHB and PLC.METHODS:The blood samples from 196 patients with CHB and 193 patients with PLC were examined to assess the presence of neutralizing antibodies against AdHu5, AdHu26 and AdC68 by adenovirus neutralization assays.RESULTS:The seroprevalence rates of neutralizing antibodies to AdHu5, AdHu26 and AdC68 in the CHB patients were 84.7%, 58.2%and 39.8%, respectively.Among the patients with PLC, the prevalence rates of neutralizing antibodies were as follows:AdHu5, 75.1%;AdHu26, 66.8%;AdC68, 32.1%.CONCLUSION:The prevalence rates and titers of neutralizing antibodies against AdC68 were the lowest among the 3 adenoviruses.There-fore, AdC68 serves as more suitable biological therapy vectors for CHB and PLC than AdHu5 and AdHu26.
